WANDERER LIST

Wandering is common for a person with dementia.  The Ozaukee County Sheriff’s Department securely maintains descriptive information and photographs of Ozaukee County residents who reside at home and are at risk for wandering.  Local police departments can access the Wanderer Registry and review the pictures and related descriptions.  If a match is found, the police can immediately contact the family or friends who are listed in the registry.

An Ozaukee County Wanderer Registry form is available at the reception window of the Mequon Police Department.  Attached to the form is an informational sheet describing the process and providing other resources for family or friends of persons with dementia. 

Family or friends of the person being registered may obtain this form by requesting it at the reception window of the police department’s lobby.  The lobby is staffed 24 hours every day, so you are welcome to pick up a form anytime.  Upon completion, you are to submit the form to the Ozaukee County Aging and Disability Resource Center, 121 W. Main Street, Port Washington, WI 53074.
